NHS vs private

Mounjaro on the NHS compared with buying privately: a factual guide

If you are considering weight-loss medication, there are two routes: the NHS, or a private or online prescription. They differ considerably in who qualifies, how long you wait, what it costs and, an aspect frequently overlooked, what support comes with it. This page sets out the position factually, so that you can make an informed choice.

The NHS route

From 2025, the NHS began offering tirzepatide (Mounjaro) through GP practices, with access being phased in, starting with the highest clinical need. In the first phase that means broadly a BMI of 40 or more and several weight-related conditions (such as type 2 diabetes, high blood pressure, high cholesterol, sleep apnoea or heart disease), with lower thresholds for people from some ethnic backgrounds. It is being rolled out gradually over several years.

Advantages: low cost, and wraparound care is built in; the NHS requires it for a minimum of nine months.

Disadvantages: most people do not yet qualify, and waits can be long.

The private / online route

Privately, the medication is available to considerably more people, considerably faster. NICE considers it clinically appropriate from a BMI of 35 with a weight-related condition[5] — a much larger group than the NHS is currently funding.

Advantages: fast, accessible, and not dependent on a phased rollout.

Disadvantages: levels of wraparound care vary between private and online prescribers, and some services provide the medication with limited monitoring, muscle protection or coming-off support. Buying from unregulated or overseas sources carries real risks. If you proceed privately, use a properly regulated UK pharmacy, and arrange the support separately if it is not included.

What both routes should have in common

NICE states the point plainly: the medication should never be provided alone[7]. Whichever route you take, wraparound care is the constant that makes treatment safe and its results lasting. If the NHS is providing your care, that provision is already in place. If you are proceeding privately, that support is precisely the gap we fill: doctor-led monitoring, muscle protection, side-effect support and a coming-off plan, around whatever prescription you have.

If you are proceeding privately

  • Buy only from a UK-regulated pharmacy
  • Ask what support is included: monitoring, blood tests, a muscle plan, a coming-off plan. If the answer is little or none, that is the gap to fill
  • Be cautious of prices that appear too good to be true, and of anything sourced from overseas or social media
