How it works
How the wraparound programme works: doctor-led support around your weight-loss medication, step by step
If you are taking weight-loss medication, or are about to start, you may not have anyone checking your blood tests, protecting your muscle, helping you manage side effects, or planning what happens when you stop. This page sets out what the programme provides: what happens, when, and why, from your first enquiry to the point at which you either maintain your results or come off the medication safely. It is built on the standard NICE and the NHS set for wraparound care, provided privately by a named doctor over nine months.[1]
Two kinds of support, working together
Clinical
Dr Chetna Patel, a certified Lifestyle Medicine Physician, oversees your care: suitability, safety, monitoring, and the key review points. We do not prescribe or change your medication; we work alongside whoever does.
Lifestyle
The practical, week-to-week support: nutrition, movement to protect muscle, help with side effects, sleep and mindset.
Many services provide an app and a rotating team. Here, the same doctor cares for you throughout.
What is wraparound care →The first step: a short enquiry
Before you pay anything, you complete a short enquiry describing your circumstances — whether you are on the NHS route, on a private prescription, still considering treatment, or already a few months in. Dr Patel reads every enquiry and we reply to tell you whether an assessment is an appropriate next step. If this is not the right service for you, we will say so.
The programme
Three phases over nine months
Phase 1: Prepare
(your first month)The purpose of Prepare is to begin with a plan rather than estimation. In an in-depth consultation with Dr Patel, we:
- review your suitability and safety, including your history, other conditions and medicines, and anything that requires care or a conversation with your prescriber;
- take a baseline, with blood tests through Helfy and body composition, so we know your starting point beyond the scale;
- assess muscle and wellbeing at the outset so they can be tracked over time;
- establish your nutrition and movement foundations, ideally before your appetite falls;
- and agree your plan and goals, including the outcomes beyond the scale that matter to you.
You leave Prepare with a clear picture of your health and a personalised plan for the months ahead.
Phase 2: Support
(months 1–6)This is the working phase, and you are not left to manage it alone. Each month includes a check-in with your doctor, with ongoing lifestyle support in between.
Clinically, each month we review how you are tolerating the medication and address side effects early, track your response and health markers rather than weight alone, and liaise with your prescriber about dose where needed.
On the lifestyle side, we work on nutrition (adequate protein and nutritionally complete meals), muscle protection (a resistance-training-first plan, because a meaningful share of weight lost can be muscle[13]), and side effects, sleep and mindset.
A typical month involves a doctor check-in, a clear focus for the weeks ahead, and support when something arises, rather than waiting to see whether you manage alone.
Phase 3: Review & Maintain
(month 6 onwards)Whatever you decide at this point, you are supported through it.
If you continue or maintain: lighter ongoing monitoring and a plan to hold your results.
If you come off: a structured, supported transition, because much of the weight lost can return within a year of stopping if nothing is in place[20]. We help you keep it off.
Checkpoint
The six-month review: a key decision point
At around six months we carry out a full review, the checkpoint NICE builds into treatment[7]. We repeat your blood tests and body composition, so we can see whether you are losing fat rather than muscle, and whether your health is genuinely improving. We then decide together what happens next: continuing, adjusting, moving towards maintenance, or beginning a planned stop. This documented review is a structured element of the programme; levels of review provision vary between online providers.
What is included
- An in-depth first consultation with Dr Patel
- Baseline and 6-month blood tests through Helfy + body-composition tracking
- Monthly doctor check-ins through the first six months
- A personalised nutrition and muscle-protection plan
- Side-effect and symptom support
- A maintenance or safe coming-off plan
How we work with your prescriber
We are non-prescribing by design. You keep your prescription from your NHS GP or a regulated pharmacy, and we provide the doctor-led care around it, liaising with your prescriber where that is helpful. This keeps our advice independent: our focus is your care, not the sale of a medication.
The NHS currently funds this medication for a small group of patients and is rolling it out gradually. If you are proceeding privately because you do not yet qualify, this programme provides the wraparound care the NHS would otherwise have provided.
